Project Hope

Project Hope Namibia in Malawi (PHN-MA) seeks qualified Malawian candidate to fill various positions for the USAID funded Ana ndi Achinyamata Patsogolo Activity (ANAPA) Project, a project aimed at preventing new HIV infections and improve the health wellbeing and Protection of Children, Adolescents, their families and Young Women (CAFYW) living with, affected by and vulnerable to HIV. ANAPA will be implemented in high HIV burdened nine districts in Malawi (Blantyre, Chikwawa, Lilongwe, Machinga, Mangochi, Mulanje, Phalombe, Thyolo and Zomba). The program focus on Orphans and Vulnerable Children (OVC) and Adolescent Girls and Young Women (AGYW) and Adolescent Boys and Young Men (ABYM). The approach is also including but not limited to System Strengthening, Continuous Quality Improvement (CQI) in close collaboration with Government of Malawi (GoM).

Community Linkage Coordinator (29)

Reporting to: Social Welfare Officer

Duty Station: PHN, ANAPA Cluster Level Office in the 8 districts of Blantyre, Chikwawa, Machinga, Mangochi, Mulanje, Phalombe, Thyolo, & Zomba.

Job Summary: S/he will be leading the planning and implementation and coordination of PHN ANAPA activities in the cluster (Catchment Areas) assigned in. Ensuring smooth linkage of CAFYW between the Clinical, KP and social partners, operations, and driving positive outcomes for project participants. Works closely with the multidisciplinary teams, government counterparts, program participants and the community at large to achieve ANAPA results.

Specific Responsibilities and Duties:

  • Develop and implement detailed site level plan

  • Collaborate with clinic staff from relevant departments (ANC, early infant diagnosis (EID)/Immunization, PMTCT), TB, ART, STI) and other Government line ministries & structures to identify and enroll the project beneficiaries.

  • Collaborate with MoH or clinical partners to jointly monitor viral load and share data on viral load for all CLHIV and their caregivers and manage those with high viral load to ensure they suppress.

  • Oversee/assist CCWs to support viral load monitoring at community level and conduct mentorship on root cause analysis and service provision for those with high viral load.

  • Support the CPWs to convene regular case conferences at clinics for CALHIV cases,

  • Foster strong collaboration with key stakeholders at cluster level while treating each facility as unique entity and strengthen site level referral linkage system.

  • Providing continuous follow-up and monitoring of new HIV-positive to support disclosure and referring for index testing.

  • Collaborate with the MEL Officer & Data Enter Clerks to implement site level CQI process.

Minimum Qualifications, expérience and required skills :

  • Diploma in health or social work-related fields, and those with bachelor’s degree will

be added advantage.

  • Minimum of 3 years of experience in HIV sensitive Child Protection Case Management, community development, Community/Clinical linkage or related field.

  • At least 1 year of experience in supervising and supporting community-based teams.

  • Motorcycle Riding license will be a plus.
